- 2-402.11 Employee hair not restrained. Employees shall wear hair restraints to effectively keep their hair from contacting the food.
- 3-602.11Ice bagged on premises without label. Food packaged on the premises must be labeled with location and address.
- 6-501.11 Damaged floor tiles, holes in the walls, missing and molded celling tiles. Kitchen, men’s restroom, and retail floor. Physical facilities shall be maintained and in good condition.
